Following documents are required for applying for renewal of GMDSS Endorsement at MMD Kolkata :-
1. Original GMDSS Endorsement (Expired)
2. Original Valid Medical Examination Report
3. Copy of GOC
4. Copy of COC
5. Copy of Passport/CDC
6. Copy of PSSR
7. Copy of AFF
8. Copy of Seatime letter from Company
(As a proof that you have sailed for minimum 1 yr in last 5 Yrs)
9. Copy of MEDICARE or Copy of MFA
10. Copy of PSCRB
11. Copy of INDOS No.
12. 2 Passport size Colour photograph.
All the copies of the valid documents as mentioned above needs to be attested prior submission. Note here that the MMD will keep your Original Medical Report so either keep a copy for later use or better still ask the doctor to give you 2 reports and use the 2nd report later if required.
Prior submission also confirm that the Seatime letter from the company bears the RPSL Number of the company.
Fees charged for this is 1000 Rs + Bank Charges.
Generally the staff will ask you to come another day for collecting but if you are in rush you can request them for the Endorsement that day itself, but of course you knew about this ....:-)
